New Password:	*	Password must contain at least one upper and one lower case alpha character, and be equal or longer than 12 charaters
Re-enter Password:		Password and confirm password should be same

NOTE: Your new password must meet the following guidelines:

• A password must consist of alphabetic/non-alphabetic characters or spaces with at least one upper and one lower case alpha character in the password.
• All passwords must consist of a minimum of twelve characters.

Once you have submitted the completed Information Form, your personal password will no longer be valid.
